Directions for Memorial Panel Quilt Submissions

Important Notes

  • This project only accepts panels memorializing people who lived in Tennessee and died by suicide or panels submitted by people who currently live in Tennessee.
  • The person requesting a quilt square must be a family member OR must have explicit consent from a close family member to make the request.
  • Please read all the instructions below, including the item about the International Suicide Memorial Wall.


Directions

  1. Find a good clear photo of the person. (If pets or other people are in the photo, we'll crop them out since the focus is only on the person, focusing on their face.) If you find several good photos and can't decide on one, you can submit them all--we'll pick the one we think will show up the best.
  2. Attach the photo to an e-mail to send to karylcb@gmail.com with PHOTO OF XXX in the subject line, filling in the name of the person for XXX.
  3. In the message box, type the full name of the person.
  4. Then, type:
  5. Their birth date, spelling out the month
  6. Their death date, again spelling out the month
  7. Next, type in the following information about you.
  8. Your name
  9. Your relationship with the person (i.e., father, sister, husband, wife, grandmother, etc.)
  10. Your e-mail address
  11. Your phone number
  12. Your mailing address
  13. Review the information you typed in steps 3 and 4 above, checking carefully to make sure there are no typos.
  14. Once you have confirmed all details are correct, send the e-mail (with the photo) to karylcb@gmail.com with QUILT - and the person's name in the subject line. After we receive this message, you will receive a reply with the word "RECEIVED" as confirmation.
  15. After we finish designing the quilt panel, we'll send you a picture of it so you can confirm that there are no mistakes. After you e-mail us back to approve the panel, we'll send it to the printer.
  16. When this year's quilt is complete, it will be presented to the TSPN central office for display at educational and awareness events around the state.


Survivors of Suicide Loss Event

TSPN typically dedicates a new quilt each year during our Survivors of Suicide Loss event, held on the first Saturday in November. Everyone who contributes a panel to the current year's quilt receives an automatic invite to the event. This year's ceremony takes place on Saturday, November 1.


International Suicide Memorial Wall

Completely separate from the memorial quilt, TSPN's central office hosts and maintains the International Suicide Memorial Wall, commemorating loved ones from across the United States and beyond. Details about the Wall, including the submission of memorial tiles, are available elsewhere on the TSPN website. You may use the same photo for both the quilt and the memorial tile honoring your loved one.
